Download a free Health At A Glance form to fill out and keep in your wallet. In an emergency, first responders will not have time to go thru your Patient Whiz, so make sure to keep a written synopsis of your health in you wallet as well.
Instructions:
1. Download the form
2. Fill it out and save it to your desktop.
3. Print page 1. Turn this page over, insert it to your printer, and print page 2 on the back of page 1.
4. Fold neatly and place in your wallet.
5. Update as often as needed.
